Martha Midlik Levandowski of West Lafayette, IN passed away peacefully on March 4, 2016 at the age of 85. She was born on April 23, 1930 in Lethbridge, Alberta, Canada to Mary and John Midlik. She was preceded in death by her husband, Dr. Donald W. Levandowski and her parents. Martha is survived by her sister and two nephews Emily Pucovsky, and Greg and John Pucovsky of Windsor, Ontario, Canada, as well as two daughters, Mari Ann Jackson (Kent) and Laura McGrew (Tom), and three grandchildren, Heather Jackson and Will and Kellie McGrew, all of Indianapolis, IN.
Martha worked as a Nurse Practitioner at the Student Hospital of Purdue University, where her husband, Dr. Levandowski, was a Professor and former Head of the Earth and Atmospheric Sciences Department. Services will be held at Church of the Blessed Sacrament on Friday, March 11 with visitation 1 hour prior to a 1:00 pm Mass officiating Father Ted Dudzinski. Burial to follow at Tippecanoe Memory Gardens. Family requests memorial contributions be given to Riley Hospital for Children care of Riley Children’s Foundation, or Don Levandowski Memorial Scholarship Fund care of Purdue Foundation.
The family thanks the Doctors, Nurses and Staff of the Forum at the Crossing for the exceptional care that Martha has received for the past.
